Physical and Dental Exams
All children entering Grade K and Grade 6 must have a recent physical on file. Students entering Grade K, Grade 3 and Grade 7 must have a recent dental exam on file.
All new students entering PreK through Grade 8 must have a physical and dental exam on file.
Injury or Illness
If a child becomes injured or ill at school, parents or the designated emergency contact will be notified. Be make sure that your contact information is up-to-date.
Medications
If your child requires a prescription or over-the-counter medication to be administered during the school day, parents must send in the medication and a signed note with the child’s name, medication, dose, route and time specified. All medication must be brought to school in its original container.
Food Allergies
Throughout the school year, parents will be notified if there is a school celebration that involves food containing an ingredient their child may be allergic to. Parents are urged to keep the school apprised of any changes to their child’s food allergy condition.
